New Motor Carrier Registration

Required Information

Basic company information: Company Name, Address, E-mail, Phone & Fax.

SCAC code – you are required to have a SCAC code setup by National Motor Freight Traffic Association.  You can apply for a SCAC code on-line at:, if your company does not already have a code assigned.

USDOT/MC number – you must show active as a carrier on Federal Motor Carrier Safety Admin. (FMCSA).  To check your status, you can go to:, put in your USDOT or MC number to pull up your account.   

TAX ID number or Employee Identification Number (EIN)

Insurance – you will need to meet ALL insurance requirements of UIIA/UIIA Equipment Providers your company wants to do business with.  Click here for the insurance requirements for each Equipment Provider.   

UIIA annual fee - either by credit card, ACH or check.  Click here to view the payment form.

Note: your company will not be charged until you become active in the UIIA system.

STEP 1 - Register (Online or by paper form).

You can apply ONLINE or you can DOWNLOAD the documents to complete & send them to our office via email: or via fax: 301-982-3414.

When applying your application online, you will be completing the following screens: 






If you have a SCAC code already listed in our system, you can log in using: SCAC code & Password
If you do not have a SCAC code set up in our system yet, you can log in using your: Account Number & Password


Once logged in, click on “Reason for Pending Application Status”.

Reasons (pop-up box) will let you know what you are missing in order to activate your UIIA account. This information is in real-time & will automatically update when the information is received.   

The example above shows that we have not received the signed Form 2 – Preamble page of the agreement. Once the Form 2 is received by our office, we will check to verify your SCAC code, your TAX ID# and the status of your companies USDOT/MC number.  The example above also shows that payment was not applied online yet & that your insurance agent has not applied your insurance information yet. 


So far you should have done the following:

-        You emailed or faxed your form 2 to our office – email: or fax: 301-982-3414.

-        You have provided your TAX ID number on the form 2 along with a fax number that is also required.

-        You have already applied for your SCAC Code on website or you already have a SCAC in place.

-        You applied your payment fee online by clicking on “manage payment method” under billing/payment info. on the left navigation bar.  Note, your payment will not be charged until your account becomes active.

-        You have forwarded the email to your insurance agent for them to apply your insurance information online. 

Next Step:

You will need to click here for a list of companies that will require additional documents to be completed by your company.   If you plan on doing business with any of these companies on the list, simply click on the Equipment Providers company name on your online account, scroll to the bottom of the page & click on “Most recent addendum-click here to download”.   If your company is not planning on doing business with any of the companies on this list, you would simply not complete the additional documents.  

Need Help: If you are having problems pulling the documents from our system, you can email our office at & request the information to be sent to you via mail.  Just let us know which company you will be needing the documents on.
